WEST VIRGINIA READING ASSOCIATION
CYNTHIA LORENTZ-COOK MEMORIAL AWARD


PURPOSE:  The Cynthia Lorentz-Cook Memorial Award was created in 1982 to honor the memory of Cynthia Lorentz-Cook, a Charleston, and West Virginia native.  The primary purpose of the fund is to provide a reservoir from which councils and members may receive assistance in the development of programs and projects, which are currently beyond their available resources.  In order to provide assistance to as many councils and members from the fund as possible, proposals requesting $300-$500 or less will be given first priority by the screening committee.

 GUIDELINES
In order to provide councils and members with as much latitude as possible in the concept and development of requests, the committee has held to a minimum the restrictions and limitations for funding.  Councils are encouraged to be imaginative and creative in the development of their requests.  Requests submitted should either be new to the council or member or be a significant expansion of a previous activity.

Councils or members receiving funding from the award will be expected to complete the project as written and to submit an evaluation form detailing the steps utilized in implementing the project.  A report, measuring the overall effectiveness of the project, will be required after the activity is carried out.  Individual member requests for funding must be submitted by a local council affiliate.

THERE ARE FOUR PARTS TO BE INCLUDED IN A PROPOSAL FOR THIS AWARD:
1. In Part I of the proposal, name the council submitting the proposal; name the person submitting the proposal along with that person’s title or position, mailing address and telephone number, and the local council affiliate.

2. Part II:  Area Of Need or Interest
List the title of the request or activity and answer the following questions:

  • What area of need will be served by the request?
  • Who will the request benefit?
  • How will the beneficiaries of the request be served?
  • 3.  Part III:  Organization and Implementation
  • What are the steps for implementation of the project?
  • What is the projected beginning date and ending date of the project?
  • How will the results of the project be evaluated?
  • 4.  Part IV:  Available Resources and Budget
    Please indicate the total anticipated budget.  In the budget, show resources that are currently available to finance your request and the amount you are requesting from the Cynthia Lorentz-Cook Memorial Fund.  Mail the completed proposal to
